A COMBINATION TECHNIQUE OF AUTOLOGOUS AUGMENTATION AND BREAST LIFTING IN HYPOPLASTIC-THIRD GRADE PTOTIC BREAST: A CASE REPORT
Breast ptosis, a condition characterized by sagging or drooping of the breasts, is a common concern among women, often caused by factors such as aging, pregnancy, breastfeeding, and weight loss. Women with ptosis often seek a solution to restore the shape and firmness of their breasts, and for many, a combination of mastopexy (breast lift) and augmentation is necessary to achieve a youthful and lifted breast appearance. This case report discusses a technique that combines these two procedures, focusing on the effectiveness of autologous tissue for augmentation in patients who wish to avoid implants.
In August 2022, a 30-year-old woman presented with third-degree breast ptosis following her second pregnancy. She expressed the desire for firmer and lifted breasts but was reluctant to use silicone implants. Due to the limited availability of fat tissue for transfer, the patient chose autoaugmented mastopexy—a procedure that uses the patient’s own tissue to achieve augmentation.
Preoperative assessment included a breast ultrasound which revealed some abnormalities. A careful preoperative design was conducted to determine the positioning of the skin and pedicles. The surgical plan involved using a wise pattern with a superior pedicle to lift the nipple-areola complex (NAC) and an inferior pedicle for the autoaugmentation procedure. The surgery was performed with attention to symmetry and the desired outcome, successfully providing the patient with lifted, firmer breasts without the use of implants.
Breast tissue undergoes significant changes during pregnancy, lactation, and hormonal shifts, which can contribute to the development of ptosis. According to the Regnault classification, the patient in this case was classified as having third-degree ptosis, the most severe form. While prosthetic implants are commonly used in breast augmentation surgeries, this patient declined that option, preferring a more natural approach.
The technique of autoaugmented mastopexy utilizes the patient’s own tissue for both lifting and enhancing breast volume, providing a natural look and feel. Unlike implants, this method avoids the risks associated with foreign objects, such as complications, infection, or rejection. Moreover, this procedure is particularly useful for patients who do not have sufficient fat for traditional lipofilling but still desire a natural result.
